Request must be typed and completed in full for faster services. Request form will not be returned. Please make a copy for your own records.
Email is the primary means of communication for requesting library resources. Please be sure the email address you provide is correct and will accept email from library@cnsu.edu. In order to protect your privacy, your email address will not be shared with a third party.
This request must be completed in one sitting. You will not be able to save information at intervals and return later. There is a time-out feature on the request. If the screen you are working in is inactive for 30 minutes, your information will be lost and you will need to restart the request process.
