Doctor of Dental Medicine (DMD)
For Fall 2021
We appreciate your interest in applying to the Doctor of Dental Medicine (DMD) Program at California Northstate University.
This application form must be fully completed and the non-refundable $100 application fee paid before applicants will be considered for admission. Comprehensive application review will occur on a rolling basis when all supporting documents have been received. This program is still under the review of the Commission on Dental Accreditation (CODA). NO APPLICATION WILL BE ACTED ON FOR ACCEPTANCE UNTIL CODA HAS APPROVED OUR PROGRAM. When the College of Dental Medicine is informed of CODA's response regarding accreditation and we are certain all steps necessary to accept our first class have been met, we will announce the accreditation status. Assuming we are granted initial accreditation, then a letter informing applicants of their acceptance status will be emailed to the address on file.

This completed application form which includes your email address that can receive email from CDM.Admissions@cnsu.edu. Notification of admissions will be sent from this email address. We recommend that you check “junk” email folders in case correspondence is inadvertently channeled to such in-boxes. Your email information will not be shared with third parties.

- completed CASPer test: visit https://takecasper.com/dates-times/ for test dates and times
- a PDF of your resumé or curriculum vitae
- An Excel spreadsheet where you have entered your prerequisite coursework and calculated your Biology/Chemistry/Physics (BCP) GPA. Link BCP-GPA-Calculation. You will upload it as an attachment at the end of your application. Please do not change the format of this excel sheet.
- unofficial transcripts from all colleges/universities you have attended
- DAT scores (You can submit your application with out DAT scores. Please submit your DAT scores by March 1 in order to be considered for admission.)
- 3 letters of recommendation
- personal statement
Admitted students who return the Enrollment Agreement will be required to provide official transcripts before beginning the DMD Program. Where possible, have transcripts sent electronically. If transcripts are mailed, have them sent to:
